- Entry is on a first come first served basis. Space is limited.
- Teams are limited to 18 players
- All players must be registered with USA Hockey
- A team roster and registration is required for all players as proof of their USA Hockey insurance
- Teams are encouraged to have one set of light colored jerseys and set of dark colored jerseys
- Team rosters and USA Hockey registration confirmation forms must be received by March 26 for 2012 tournament
- All participants are required to sign a release of liability form
